Reference Range
Interpretative guidelines are based on the July 2012 California Department of Public Health recommendations for the follow-up of a positive newborn screen for primary congenital hypothyroidism
1. If the Newborn Screening results for TSH > 40
µIU/mL: Repeat TSH and test for Free T4. Neonate may
require immediate treatment. Refer neonate to pediatric
endocrinologist promptly.
2. If the Newborn Screening results for TSH were between 29- 40
µIU/mL: Repeat TSH and test for Free T4 level.
Treatment should be held until confirmation tests are
completed.
a. If the repeat TSH was collected
during the first 2 weeks of life and the repeat TSH result is ≤
10 µIU/mL, the result is normal and the case can be resolved
as negative.
b. If the repeat TSH results are ≥ 10 but ≤ 40 µIU/mL,
repeat the TSH and Free T4 again. Refer neonate to pediatric
endocrinologist who will interpret the TSH results in conjunction
with Free T4 values.
c. If the repeat TSH result is ≥ 40 µIU/mL, neonate may
require immediate treatment. Refer neonate to pediatric
endocrinologist promptly.
